AN ACT

Amending the act of April 9, 1929 (P.L.177, No.175), entitled

2

"An act providing for and reorganizing the conduct of the

3

executive and administrative work of the Commonwealth by the

4

Executive Department thereof and the administrative

5

departments, boards, commissions, and officers thereof,

6

including the boards of trustees of State Normal Schools, or

7

Teachers Colleges; abolishing, creating, reorganizing or

8

authorizing the reorganization of certain administrative

9

departments, boards, and commissions; defining the powers and

10

duties of the Governor and other executive and administrative

11

officers, and of the several administrative departments,

12

boards, commissions, and officers; fixing the salaries of the

13

Governor, Lieutenant Governor, and certain other executive

14

and administrative officers; providing for the appointment of

15

certain administrative officers, and of all deputies and

16

other assistants and employes in certain departments, boards,

17

and commissions; and prescribing the manner in which the

18

number and compensation of the deputies and all other

19

assistants and employes of certain departments, boards and

20

commissions shall be determined," further providing for fees

21

chargeable by Department of State; and providing for a

22

uniform form.

The General Assembly of the Commonwealth of Pennsylvania

24

hereby enacts as follows:

25

Section 1.  Section 618-A(1) of the act of April 9, 1929

26

(P.L.177, No.175), known as The Administrative Code of 1929, is

27

amended by adding a subparagraph to read:

Section 618-A.  Department of State.--The Department of State

is authorized to charge fees for the following purposes and in

2

the following amounts:

3

(1)  Bureau of Commissions, Elections

4

     and Legislation:

5

(x)  Fee for authentication of a document

7

             for recognition by a foreign government

8

             (apostille) or certification required in

9

             an international adoption proceeding...5.00

Section 2.  Within 30 days of the effective date of this

12

section, the Department of State shall make available and

13

publish in the Pennsylvania Bulletin a uniform form to request

14

for the legalization of documents. The form shall include all of

15

the following:

16

(1)  Name of requester.

17

(2)  Organization.

18

(3)  Address.

19

(4)  Telephone number.

20

(5)  Country for which documents are being prepared.

21

(6)  Number of documents to be certified.

22

(7)  Whether the request is for legalization of documents

23

for an international adoption proceeding.

24

(8)  Signature of requester.

25

Section 3.  This act shall take effect immediately.
