(2) The name and address of the chief operating officer
and medical director.
(3) The signature of the secretary.
(i) Statement of grounds for denial of license.--If the
department does not issue a license to an applicant within six
months after the filing of an application, the license shall be
deemed denied. When an application is denied or deemed denied,
the department shall, within seven days, state the grounds and
reason for its refusal, in writing, with a copy to the
applicant.
(j) Hearings and appeals.--If an applicant is denied a
license, the applicant may request and receive a hearing before
the secretary, if the hearing request is made within 30 days
after receiving notice of denial.
(k) Revocation of license.--
(1) A license may be suspended or revoked by the
department for a violation of any provision of this act.
(2) Before the department suspends or revokes a license,
written notification shall be provided to the licensee. The
notification shall specify the reason for revocation or
suspension and indicate a time and place for a hearing on the
matter, to be held within 30 days of notification.
(l) Ongoing review.--The department shall conduct an ongoing
review of the clinical guidelines issued by the American Academy
of Pediatrics regarding the assessment and determination of
medical necessity for the use of donor milk to treat medically
fragile infants and children.
Section 7. Licensure fees.
In order to obtain licensure by the department, an applicant
shall submit an initial licensure fee of $1,000, with a $250 fee
